Horse Registration

Adoption Forms

Once you have decided to adopt a horse through the SAQ, you can find the forms and procedures to finalise the process here:

a. Please fill in the Adoption Information Pack and Application Form

b. Don't forget to fill in the Membership Application Form

Completed forms and membership fees should be sent to:

Horse Placement Officer

or emailed to the SAQ Horse Placement Officer

c. Once a suitable horse is found, you will need to sign an Horse Placement Program Agreement and a Vet/Health Check Agreement and these documents are legally binding.

After 3 months a check of the horse will be required. The purpose of your 3 month check is to try and ensure the adoptee of the horse is coping with the responsibilities of owning a horse and that the horse is not being neglected or mistreated. 

Please fill in the First Wellbeing Check Form. 

d. Part of the requirements for people obtaining horses through this adoption program is to have the horse ‘wellbeing checked’ by a vet at 6 months and again at 12 months.

  1. After   6 Months: Please send through the 6 month Wellbeing Check Form
  2. After 12 Months: Please send through the 12 month Wellbeing Check Form
